It applies to website visitors and to communications initiated through the telephone and email links on this website. Information you choose to provide
The website does not presently provide an online intake form, patient portal, user account, or online payment function. If you call or email the office, you may voluntarily provide your name, telephone number, email address, the reason for your inquiry, and other information needed to respond. Please do not send detailed medical records, imaging files, Social Security numbers, payment-card information, or other highly sensitive information by ordinary email unless the office has specifically instructed you to use an appropriate method.
Information processed when the website is visited
The website hosting and security services may automatically process limited technical information such as an Internet Protocol address, browser and device type, requested page, date and time, and security or error information. This information is used to deliver, protect, maintain, and troubleshoot the website. The website is not presently configured with advertising trackers or a system for selling or sharing visitor information for cross-context behavioral advertising.
